PREPARE FOR YOUR APPOINTMENT

When you come for your appointment, please bring the following:

• Driver’s License or a Valid Photo ID
• Insurance Card(s)
• Insurance Co-payment, co-insurance
• PCP Referral (if required)
  • Please verify that our office has received the referral prior to your appointment
• Images - If you’ve had any imaging performed previously, you can either send it to us through this link below, or you can bring the disc with you to the appointment
  • Click here to create an account with our Imaging software system (TeleRay) so you can drag and drop your DICOM images to us prior to your appointment
    • Destination: Boston Sports and Biologics

• Previous Imaging Reports: X-rays, MRIs, CT Scans, etc. or any other relevant information to your injury
• Consultation Notes from the provider who referred you to see us (if applicable)

You will receive a text/email reminder of your appointment along with a link to complete your online registration. This registration is mandatory to ensure we have accurate billing and personal information.
If you do not complete it before your visit, you will be asked to finish it in the waiting room prior to your appointment.

NEW patients

Please print and complete the new patient forms and bring them to your first appointment. These forms are separate from the online registration link and include questions about your injury or the reason for your visit.

If you cannot complete the forms ahead of time, please arrive 15–20 minutes early to complete them in the waiting room.
